Marriage Application Requirement Oregon: Get Copy Of Birth Certificate

To complete the application for a marriage license, a couple must:

The following information is required on the marriage license for each party to the marriage:

  • Address

  • Date of Birth

  • Place of Birth

  • Father’s Name

  • Father’s place of birth

  • Mother’s Maiden Name

  • Mother’s place of birth

Name Change:
